Centralized Plan Coordinator

Full-Time Openings

The Plan Coordinator is responsible for overseeing the plan of services based on person centered planning concepts that are delivered to each assigned individual. The Plan Coordinator works with the person and their support team to develop and revise the habilitation goals with a focus on areas that are most important to the person, and is ultimately responsible for ensuring consistent implementation and documentation of all aspects of the individual's program plan. The Plan Coordinator develops these services in compliance with all applicable state and federal regulations. The Plan Coordinator reports directly to the Plan Coordination Manager.

Requirements

  1. Develop Habilitation Plan annually for each individual using person centered planning, along with corresponding documentation mechanisms as necessary and review semi-annually.
  2. Attend annual and semi-annual review meetings for each individual assigned. Ensures appropriate follow up on recommendations.
  3. Works closely with the person and their support team to ensure services, health related and otherwise, are implemented appropriately.
  4. Develops monthly summaries for services each person receives. Analyze data and determine progress/ regression, etc. Revises program plan as needed.
  5. Participates in decisions-making process related to people assigned as appropriate.
  6. Conduct on-site observations and provide feedback to staff to ensure proper, consistent implementation of goals and plans.
  7. Provide input into the development of behavior support plans as appropriate for individuals.
  8. Develops plan and works with divisions in ensuring all information necessary for new intakes is obtained, implemented and shared appropriately with team members.
  9. Participates in the Personal Outcome Measures interview process, including but not limited to completing POM interviews, providing information to interviewers, etc. Incorporates individual's POM results into their habilitation plans as appropriate
  10. Possess effective interpersonal and communication skills to handle sensitive issues with individuals and the people who support them.
  11. Maintain open communication with Medicaid Service Coordinators, families and program staff.
  12. Provide training to staff, with assistance of divisional supervisors and/or agency nursing, clinical staff, on all aspects of an individual's program plan including goals, behavior support/monitoring plans, special diets and all corresponding documentation.
  13. Attend staff meetings as appropriate and provide training to staff regarding implementation of all aspects of the plan.
  14. Participation in the quality assurance process as assigned by the Plan Coordination Manager.
  15. Maintains individual files and record keeping procedures for quality and compliance in accordance with OPWDD as well as local, state and federal regulations.
  16. Ensures the timely completion of required assessments, monthly summaries and habilitation plans in accordance with regulatory requirements and agency policies. Ensures habilitation plans are sent to MSC's within the required regulatory time frame.
  17. Establishes and provides clear and legible documentation and compliance records required by Arc and other government agencies. Expected to conduct all agency business and complete job responsibilities in accordance with agency Code of Conduct and Corporate Compliance Plan
  18. Must be able to travel to various program locations on a regular schedule to provide ongoing oversight to the implementation of the individual's habilitation plan. Responsible for providing safe transportation as necessary or required to individuals to and from off-site locations in agency-owned vehicle and/or own vehicle.
  19. Supports the Goals and Mission of Arc of Onondaga. Ensures that your communications and personal performance reflect the Agency's commitment to teamwork, compliance with policies and procedures and respect for participants and staff.
  20. Computer and office technology expertise that is required to fulfill job responsibilities and assignments.
  21. Attends and participates in required Arc meetings, staff development and training programs. 
  22. Provides other job-related support and services as required or assigned by management

Education & Experience Requirements

  • Bachelor's Degree in Human Service field and one year experience of working with individuals with developmental disabilities.
  • Meet QIDP eligibility requirements
  • A valid NYS driver's license and a driving record acceptable to the Agency's insurance carrier.

Physical Demands & Working Conditions

Work requires minor physical exertion with incidental or no adverse environmental conditions. The need for physical stamina, endurance, and physical manipulation are limited. Body movement usually involves sitting and intermittent walking and standing.
