The LPN is responsible for assisting in the provision of health care and nursing services to individuals residing in an Arc of Onondaga residence and/or attending Day Habilitation, including promotion and maintenance of health, and prevention of illness and disability. The LPN is responsible for assisting the RN with oversight and assigned duties associated with a caseload, including incorporating personal goals into a daily program of activities. This position is responsible for providing hands-on assistance, supervision, instruction, and guidance. Reports directly to the Nurse Manager and follows the direction of the assigned Team Registered Nurse.


  1. Assists the assigned RN with Providing emergency medical care or first aid treatment in the event of illness, injury and/or accident. Administers medication, tube feedings, injectable medications and medical treatments to individuals as needed. Monitors medication and side effects. Observes all medication policies and procedures
  2. Provides input into the development and implementation of nursing care plans for individuals.
  3. Assists in providing education and instruction to individuals supported and staff to ensure the provision of health care services, including activities of daily living, personal hygiene, self-medication, oral hygiene and health education or any other topics assigned by RN.
  4. Maintains regular communications with families and medical staff from other agencies.
  5. Notifies care providers of need for medical follow-up by written and telephone contact, informing all team members of same.
  6. At times may observe, assist, supervise, and instruct individuals during meals and at other times as assigned/requested. Assists, at any time, with physical and/or behavioral interventions, including dining, transferring, lifting, and positioning, demonstrating appropriate techniques in all interactions. May provide coverage at a location that requires LPN licensure.
  7. Assists the RN with the following duties including but not limited to: setting up and reviewing MAR's, reviewing medication errors, preparing review paperwork, training staff on documentation standards as it relates to medical, filing medical paperwork, updating IDF's, preparation of Nursing Care plans for RN approval.
  8. Responsible for Medication Cabinet Checks as assigned, including completion and submission of appropriate form to the RN Manager and appropriate parties. Assists in ordering of first aid and other medical or health related supplies, as requested. Responsible for the safe storage and disposal of supplies.
  9. Acts as a member of the treatment team, providing feedback, suggestions, etc. for program plans for individuals. Assists, as requested, in the evaluation of individual's needs and abilities. Attends annual and semi-annual reviews to present medical information.
  10. Holds flexibility in scheduling to meet the needs of the people supported and programing
  11. Provide "back-up" coverage including plan development and review, documentation and support for individuals other than those assigned to their caseload when necessary.
  12. Responsible for providing safe transportation as required to self and to individuals to and from off-site locations in agency-owned and/or personal vehicle. This position requires a fair amount of traveling between assigned locations.
  13. Serves on program- and agency-wide committees, as requested.
  14. Maintains and updates all necessary documentation, health tracking report, nursing care plans, Therap, specified on the monthly nursing checklist.
  15. Responsible for completing informed consents and getting signatures.
  16. Works closely with residential staff to ensure that the medical appointment record is complete, as well as providing overall oversight of medical appointments ensuring that they are scheduled and attended by the individuals.
  17. May be requested, as needed, to provide services to any division within the agency lending support and expertise where required.
  18. Supports the Goals and Mission of Arc of Onondaga County. Ensures that communications and personal performance reflect the Agency's commitment to teamwork, compliance with policies and procedures and respect for individuals and staff.
  19. Expected to conduct all agency business and complete job responsibilities in accordance with agency Code of Conduct and Corporate Compliance Plan.
  20. Attends and participates in required Arc meetings, staff development and training programs.
  21. Establishes and provides clear and legible documentation and compliance records required by Arc and other government agencies.
  22. Must maintain any required licenses and/or certifications.
  23. Provides other job-related support and services as required or assigned by management.

Education & Experience Requirements

  • Currently licensed and registered as a Licensed Practical Nurse by the NYS Department of Education.
  • One year in training or working with the developmentally disabled or specialized training, preferred.
  • A Valid NYS driver's license and a driving record acceptable to the Agency's insurance carrier.

The job requires specific levels of responsibility for the safety of the incumbent and people interacted with on a daily basis. Safety requirements and decisions reflect agency or legal requirements that are above and beyond generally accepted practices and procedures for personal safety, the safety of other staff, and includes specific direct supervision and care accountabilities for the safety of individuals.

Physical Demands & Working Conditions

Work requires routine physical exertion equivalent to lifting between 25 and 50 pounds, for approximately 25% - 75% for their workday, while assisting, individuals, moving materials and/or operating equipment. Incumbents are exposed to undesirable or unpleasant environmental characteristics. Position requirements for physical stamina, endurance, and physical manipulation are moderate. Accountabilities require sustained amounts of time sitting, walking and/or standing. The use of some safety equipment and/or routine precautions is required.

Responsibility for Resources

The job requires very limited responsibility for resources. Examples of resources include personal work materials, supplies or equipment, or less than $50 cash. The staff member has very limited or indirect control over these resources. The variety and volume of resources is also limited. Problems and decisions associated with resources are standard with limited impact beyond the incumbent's primary job accountabilities.
