Registered Nurse - Case Manager

The Nurse is responsible for identifying the medical and health care needs of residents and monitoring compliance with service and treatment plans across program settings. Responsibilities include developing nursing care treatment plans, monitoring the administration of medications and documenting adherence and follow-up to ensure appropriate medical care for individuals supported. The incumbent provides training to direct care staff (day and residential services) regarding the administration of medications and facilitating the delivery of required medical health care services. The Nurse may provide direct assistance and support to residents to secure required medical care and services.


1. Develops nursing care plans for residents and other programs as needed and maintains current documentation for prescribed medications and health care services.

2. Oversees various agency trainings for assigned caseload and follow through with the certification process for medication administration, insulin administration and tube feeing/ medication certifications. Provides oversight to the AMAPs.

3. Completes monthly nursing notes.

4. Assists in assessing dietary needs, including dietary counseling, monitoring meals, special dietary needs and weights.

5. Provides effective guidance, counseling, training and communications (both oral and written) to Individuals and direct care staff to ensure compliance with nursing care plans and the administration of medications.

6. Reviews and updates the health care status of Individual's and implement required changes. Initiates and documents monthly reviews of residents' and program participants medication administration records.

7. Establishes effective and collaborative working relationships with clinical and direct service providers both inside and outside of Arc.

8. Maintains current information related to NYS and Federal regulations and guidelines related to the medical and health care requirements for people with developmental disabilities.

9. Conducts at least weekly site visits to individual's residences and programs sites within Arc of Onondaga) to confirm accurate and safe administration of medications. Confirm security and safety requirements for the storage of medications.

10. Maintains and updates Semiannual/Annual review paperwork ensuring that services are individualized and designed to promote independence. RN will attend these meetings to ensure accuracy and consistency amongst divisions

11. Assist in ensuring that infection control policies and practices are implemented.

12. Provides instruction and supervision to staff to ensure the provision of health care services.

13. Provides emergency medical care or first aid treatment in the event of illness, injury or accident.

14. Follows all medication/ health-related policies and procedures.

15. Provides direction to assigned LPN.

16. Provide "back-up" coverage including plan development and review, documentation and support for individuals other than those assigned to their caseload when necessary.

17. Ability to drive to assigned home locations in Onondaga County and accompanies residents on health care appointments and testing as needed.

18. Owns or leases a reliable automobile to provide transportation required for this position. The automobile must be available to provide safe transportation for self, residents and/or staff.

19. Must maintain job-required certifications and/or licenses.

20. Must carry an Agency provided cell phone at all times during normal work hours and provide after hour coverage on a rotational basis.

21. Participates in and /or chair committees as assigned by the Nursing Supervisor.

22. Must be flexible to assess medical situations and/or injuries on off-hours at program sites.

23. Available to attend consumer's 6 month and annual reviews.

24. Supports the Goals and Mission of Arc of Onondaga County. Ensures that your communications and personal performance reflect the Agency's commitment to teamwork, compliance with policies and procedures and respect for residents and staff.

25. Computer and office technology expertise that is required to fulfill job responsibilities and assignments.

26. Attends and participates in required Arc meetings, staff development and training programs.

27. Establishes and provides clear and legible documentation and compliance records required by Arc and other government agencies.

28. Expected to conduct all agency business and complete job responsibilities in accordance with agency Code of Conduct and Corporate Compliance Plan.

29. Provides other job-related support and services as required or assigned by management.

Education & Experience Requirements

  • Academic Degree in Nursing plus Certification as a Registered Nurse.
  • Certified in CPR
  • Preferred one year experience with developmental disabilities population and case management experience.
  • Combination of education and experience which, in the judgment of the employer, equals above standards.
  • A valid NYS driver's license.

Physical Demands & Working Conditions

Work requires routine physical exertion equivalent to lifting between 25 and 50 pounds, for approximately 75% of their workday, while assisting individuals, moving materials and/or operating equipment. Incumbents have moderate exposure to undesirable or adverse environmental conditions. Position requirements for physical stamina, endurance, and physical manipulation are significant. Accountabilities require sustained amounts of time sitting, walking and/or standing. The use of safety equipment and/or specific precautions is required.
