Program Manager

Full-Time position

The Day Habilitation Program Manager is responsible for the day to day site operation of the Day Habilitation Program.  Must have working knowledge of HCBS Waiver. Reports to the Director of Community Supports.

Requirements

  • Hires, supervises and trains direct care staff including completion of annual performance evaluations.
  • Assures that all required documentation is current in individual's files.
  • Reviews and approves all Day Habilitation Plans for site individuals to ensure compliance with their valued outcomes and the 3IP philosophy.
  • Attends and participates in all required program training, planning meeting and standing committees.
  • Acts in the capacity of Agency designated supervisor for medication administration oversight at the program site.
  • Directs staff to maintain principles of service delivery including OPWDD's Five Governing Principles and the 3IP philosophy.
  • Develops and implements organized methods for scheduling, monitoring and evaluating appropriateness of individual's activity patterns.
  • Monitors program site expenses and maintains all supplies and inventories.
  • Assumes primary responsibility for monitoring the site's physical space including the completion of safety checklists, issuing repair orders, etc.
  • Establishes and maintains positive relationships and rapport with families, caregivers, community organizations and other human service agencies. Acts as the liaison between the program and caregivers, etc.
  • Serves as a back up for direct care staff in case of emergency, staff shortages, etc.
  • Responsible for the completion of DDP-1 & DDP-2 forms for all site individuals.
  • Prepares and submits all required reports, statistics and quality assurance reviews.
  • Monitors and reviews all general event reports and incidents and develops systems to ensure the health and safety of individuals at program site.
  • Must be able to transport self and individuals in an Agency provided vehicle and/or own vehicle.
  • Must maintain any required licenses and/or certifications.
  • Supports the Goals and Mission of Arc of Onondaga County. Ensures that your communications and personal performance reflect the Agency's commitment to teamwork, compliance with policies and procedures and respect for consumers and staff.
  • Attends and participates in required Arc meetings, staff development and training programs.
  • Establishes and provides clear and legible documentation and compliance records required by Arc and other government agencies.
  • Performs investigations related to serious and serious reportable incidents as required or directed by supervisor.
  • Must be able to work flexible hours.
  • Expected to conduct all agency business and complete job responsibilities in accordance with agency Code of Conduct and Corporate Compliance Plan.
  • Provides other job-related support and services as required or assigned by management.

Education & Experience Requirements

  • Bachelor's degree in Human Services or other related field.
  • 2 years pertinent experience.
  • Combination of education and experience which, in the judgement of the employer, equals above standards.
  • A valid NYS driver's license and a driving record acceptable to the Agency's insurance carrier.

Physical Demands & Working Conditions

Work requires occasional equivalent to lifting up to 25 pounds and limited exposure to some undesirable environmental characteristics.  The position requires the need for physical stamina, endurance, and physical manipulation that can be somewhat taxing.  Accountabilities routinely require considerable amounts of time sitting, walking and/or standing.  The use of some safety equipment and/or routine precautions may be required.
